Patient Care Coordinator – Home Health
The Patient Care Coordinator assumes the responsibility to schedule patient services and to coordinate their care staffing in accordance with state and federal guidelines.
Job Qualifications
Education: Graduate of an accredited High School.
Experience: One-year experience in a clinical setting, Home Health preferred.
Skills: Good communication, interpersonal and organizational skills.
Requirement: Reliable transportation and valid auto liability insurance.
Work Environment: Works in an office environment; Noise level may be moderately high at times; The ability to work a flexible schedule and extended hours; The ability to travel locally and some exposure to inclement weather.
Job Specific Efforts Required: Periodic times of prolonged standing, sitting and walking; working under conditions necessary to meet patient’s needs and program time-frames; timely decision making and resource acquisition; requires hand-eye coordination and manual dexterity.
Essential Functions of the Patient Care Coordinator include but are not limited to:
Responsible for scheduling agency patient visits.
Assess, plan, and coordinate changes in patient care services. Responsible for the supply needs of all patients assigned.
Assess and verify that all Nurses, Aids and other disciplines are in compliance with documentation (timely submission and QA) and that the daily schedule is being followed.
Serves as liaison between patients, caregivers, doctors and doctor’s office staff, agency administrative staff and contracted services.
Provides clinical and staff feedback as needed.
Assist the Director of Nursing in certification and recertification process.
Establishes, monitors and evaluates productivity standards by conducting chart audits for completing of documentation on a routine basis.
Consults with all members of the administrative and professional staff to coordinate goal oriented, quality nursing and medical care.
Assists with marketing the Agency and its programs with the opportunity arises.
Performs other functions as assigned by the Director of Nursing.
