We are seeking a compassionate and skilled Registered Nurses to provide quality in home nursing care for patients. In this role, you will coordinate patient care, perform skilled nursing procedures, and work closely with an interdisciplinary team to ensure the best possible outcomes.


KEY RESPONSIBILITIES

- Conduct initial and ongoing patient assessments

- Develop, implement, and update individualized plans of care

- Provide direct skilled nursing care as ordered by physicians

- Educate patients and caregivers on health needs and procedures

- Communicate changes in patient condition to providers and care team

- Complete clinical documentation accurately and timely

- Participate in oncall rotation as assigned

- Ensure compliance with infection control and agency policies

- Uphold agency policies and professional standards.


MINIMUM REQUIREMENTS

- Graduate of an accredited diploma, associate, or bachelor nursing program

- Current Registered Nurse (RN) state license.

- At least 1 year of clinical nursing experience; home health preferred

- Current CPR certification

- Valid driver’s license, reliable transportation, and auto insurance

- Strong interpersonal and communication skills


WORK ENVIRONMENT

- Care provided in patient homes under varying conditions.

- Exposure to infectious diseases, blood, and bodily fluids possible.

- Requires local travel and ability to work a flexible schedule.

- May involve prolonged standing, walking, and lifting up to 50 lbs.
