We are seeking a compassionate and skilled Licensed Practical/Vocational Nurse to provide hospice nursing care in patients’ homes. This role supports the RN and interdisciplinary team to deliver high quality end of life care for patients.
KEY RESPONSIBLITIES
- Deliver skilled nursing care in alignment with the patient’s plan of care.
- Assist in assessing physical, psychosocial, and environmental patient needs.
- Participate in developing and implementing patient care plans.
- Ensure continuity and quality of care through timely documentation and teamwork.
- Communicate effectively with patients, families, team members, and referral sources.
- Participate in interdisciplinary meetings, in‑service training, and quality improvement programs.
- Follow infection control and safety procedures.
- Uphold agency policies and professional standards.
MINIMUM REQUIREMENTS
- Graduate of an accredited LPN/LVN program.
- Current LPN/LVN state license
- Current CPR certification
- Valid driver’s license, reliable transportation, and current auto insurance.
- Strong clinical and interpersonal skills.
- Minimum 2 years of clinical nursing experience; hospice experience preferred.
WORK ENVIRONMENT
- Care provided in patient homes under varying conditions.
- Exposure to infectious diseases, blood, and bodily fluids possible.
- Requires local travel and ability to work a flexible schedule.
- May involve prolonged standing, walking, and lifting up to 50 lbs.
