We are seeking a compassionate and skilled Registered Nurse to provide hospice care to patients in their homes. This role coordinates care with an interdisciplinary team, supports patients and families, and ensures high‑quality end‑of‑life care.


KEY RESPONSIBILITIES

- Conduct initial and ongoing comprehensive patient assessments ensuring hospice eligibility criteria is met

- Develop, implement, and revise individualized plans of care.

- Educate patients and families on disease processes, self-care, nutrition, and endoflife needs.

- Coordinate care with physicians, caregivers, and interdisciplinary team members.

- Provide clinical direction to Hospice Aides and LPNs/LVNs.

- Maintain accurate, timely documentation of care and progress.

- Participate in oncall rotation and respond to patient needs promptly.

- Follow infection control and safety procedures

- Uphold agency policies and professional standards.


MINIMUM REQUIREMENTS

- Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s).

- Current Registered Nurse (RN) state license.

- Current CPR certification.

- Valid driver’s license, reliable transportation, and auto insurance

- Strong communication and interpersonal skills.

- Two years of clinical experience required; hospice experience preferred.


WORK ENVIRONMENT

- Care provided in patient homes under varying conditions.

- Exposure to infectious diseases, blood, and bodily fluids possible.

- Requires local travel and ability to work a flexible schedule.

- May involve prolonged standing, walking, and lifting up to 50 lbs.
