We are seeking a dedicated and experienced Weekend On-Call Registered Nurse to join our healthcare team. The ideal candidate will possess a strong background in Skilled Home Health services and will be responsible for patient visits after hours and on weekends. This role requires excellent communication skills, a deep understanding of patient needs, and the ability to coordinate care effectively across multiple disciplines.
Availability required: 5 pm on Friday through 8 am on Monday.
KEY RESPONSIBILITIES
- Conduct initial and ongoing patient assessments
- Develop, implement, and update individualized plans of care
- Provide direct skilled nursing care as ordered by physicians
- Educate patients and caregivers on health needs and procedures
- Communicate changes in patient condition to providers and care team
- Complete clinical documentation accurately and timely
- Participate in on‑call rotation as assigned
- Ensure compliance with infection control and agency policies
- Uphold agency policies and professional standards.
MINUMUM REQUIREMENTS
- Graduate of an accredited nursing program (Diploma, Associate, or Bachelor's)
- Current Registered Nurse (RN) state license.
- 2 years of clinical experience required; home health preferred
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Strong interpersonal and communication skills
WORK ENVIRONMENT
- Care provided in patient homes under varying conditions.
- Exposure to infectious diseases, blood, and bodily fluids possible.
- Requires local travel and ability to work a flexible schedule.
- May involve prolonged standing, walking, and lifting up to 50 lbs.
