The primary function of the Hospice Chaplain is to provide spiritual care and support in the home to terminally ill patients and their families. The Chaplain provides an assessment of the patient’s and family’s spiritual needs and provide counseling to meet those needs. And the Chaplain is a core member of the Interdisciplinary Group.


Job Qualifications


Education: Master of Divinity preferred.


Licensure: Per State guidelines if any.

Experience: One (1) year of experience as a Chaplain in a healthcare institution preferred.


Skills: Working knowledge of community spiritual and other resources. Good interpersonal skills.



Transportation: Reliable transportation. Current auto liability insurance and driver’s license if personal vehicle is used for work.



Environmental and Working Conditions: Works in patient homes and other healthcare facilities under various conditions. Occasional exposure to infectious diseases; ability to travel locally; may have some exposure to unpleasant weather.



Physical and Mental Effort: Prolonged standing, walking, and sitting required. Requires working under some occasionally stressful conditions to meet deadlines and patient/family needs, must be able to make quick decisions; functions as a facilitator to meet patient/family spiritual needs.


Essential Functions of the Hospice Chaplain include but are not limited to:


  • Assess the Hospice patient and support system to identify spiritual needs as evidenced by POC, documentation in the clinical records, and IDG/T conference minutes. Provide support for the spiritual needs of patient/family.

  • Attend Interdisciplinary Group/Team meetings and participate in the patient care planning process.

  • Participate in Quality Assessment Performance Improvement process as required.

  • Use effective interpersonal relations and communication skills, facilitates the use of these skills by other team members to achieve desirable outcomes.Meets mandatory continuing education requirements of the Hospice.

  • Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements.

  • Promotes the Hospice’s philosophy and administrative policies to ensure quality of care.

  • Demonstrates understanding of coordination and continuity of care by collaborating with the IDG/T.

  • Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
